Submission Information

Please ensure that all fields are filled in and the required photo and release form must be uploaded prior to clicking submit.  Be advised that if the fields are not filled in or the required information is not uploaded, the system will not accept your application.

Eligibility Criteria Checklist

To be eligible for participating with Selection Jamaica Olympic Program, all players must meet the following participation criteria:

Native (Born) Jamaican or of Jamaican Descent

Born on or after January 1, 1997

Age 17 through 23

Parental Permission for age 17

Currently in the United States

Required I.D. for Proof of Age 

Select only one location to apply for.

(*Each player only allotted one location)

All applicants are required to upload a passport photo as a part of the application process.
